I doubt Moriarty is still alive. Sherlock would've made sure he was really dead before deciding to fake his own death, and Moriarty would've known that Sherlock would check his breathing at least. Besides, from a narrative standpoint, the show would have no stakes or trust in the audience if it turned out both characters we saw die were alive. There has to be actual consequences. I think maybe one of Moriarty's goons broadcasted the message, and Moriarty left an ongoing plan in place for others to carry out after he died, or maybe you're right and it was Sherlock's brother.
